Programme Forms

In this section we will go through the various programme information forms within PAC.

Available Forms

The forms available to you will depend on the liquidated damages required by the contract. There exact location will depend on whether they are linked to a series and/or one of it's episodes or a single programme. For a typical series, the following screenshot shows a representative sample of what forms will be present for the series and it's episodes.

Single Programme

For a single one off programme, the following forms will typically be available.

  • Deliverables - List of required deliverables and their current status
  • Contributors - List of non-union contributors who are part of the programme
  • Artists - List of union artists who are part of the programme
  • 405 Prog Del Form - Record of the delivery of the three forms it contains
    • Presentation Requirements
    • Content Advice Phrases
    • Advertising Data
  • 405b Synopses and Billing
  • 406 Subtitles - Name of the company providing the subtitles
  • 408 Music Cue Sheet - Music cues for the programme sound track

Series

For a series, the following forms will typically be available.

  • Music Cue Sheet Templates - Template cue sheet entries for works used multiple time in the series
  • Deliverables - List of required deliverables and their current status
  • Contributors - List of non-union contributors who are part of the programme
  • Artists - List of union artists who are part of the programme

Series Episodes

For each episode of a series, the following forms will typically be available.

  • Deliverables - List of required deliverables and their current status
  • 405 Prog Del Form - Record of the delivery of the three forms it contains
    • Presentation Requirements
    • Content Advice Phrases
    • Advertising Data
  • 405b Synopses and Billing
  • 406 Subtitles - Name of the company providing the subtitles
  • 408 Music Cue Sheet - Music cues for the programme sound track
